In order to be eligible for this, you need to fill out the application form and return it to jcmiller4@gmail.com.

Deadline to return the form is Sunday, February 20,2011.

Our next meeting is on February 21st and at that time, the board will review the applications and decide how much we can allot to each family requesting the use of the funds based on evenly dividing this set amount and then one of us will contact you. If you do not know of specific programs right now but wish to be considered for the program, state that on the application and still return it because we cannot accept applications beyond this deadline since this amount of money is fixed and will not fit into our budget at a later date. We will let you know the amount and then the basic reimbursement policy will take place. You pay for the class or program and then mail me a receipt and I will then reimburse you (as long as you have board approval for the class/program ahead of time).

You can email me the completed application or print it, complete it, and mail it to 352 McBath Street, State College, PA 16801. All individuals are welcome and encouraged to take part in this from birth through adulthood. The CCDSS would like the opportunity to "enrich" each individual's life in some way. In addition, if you prefer to opt out of this program, that too is fine, but we want everyone to know the opportunity is available to them.

Please note that those individuals that were part of therapeutic horseback riding in the past will be required to submit this form in order to be covered for that program from now on.
